1) The user that is experiencing/reported the issue. (If it's a specific user that is experiencing the issue, please note this down)

2) A URL to the page you are having issues with.
(If template issue, both a URL to the folder of the main template and a direct template to the document itself or the name of the created document)

3) Steps to reproduce the issue. (A step-by-step explanation of how to recreate/experience the problem)

4) A short description of the problem with a screenshot or a video highlighting the issue. (Describe the problem as short as possible, use screenshots or video, and highlight the error or bug)
